Rates
Please contact me for our current rates, which depend on location and travel distance. Once booked, an independent contractor agreement, practice questionnaire and W-9 form will be emailed or mailed to you.
Booking Minimum
My booking minimum is eight hours (waived on Saturdays and Sundays). The maximum length for an unpaid lunch is one hour. Early dismissal is not possible if the hours worked falls under the booking minimum.
Cancellation Policy
I require a 30 day cancellation notice, however there is no fee for cancellations outside of the 30 day notice. Because reliability is important to our relationship, cancellations within 30 days will incur a fee of 50% of the expected total booking hours. Cancellations within 14 days will incur a fee of 75% of the expected total booking hours.
Invoice Due Date
An invoice will be emailed or mailed to you within 48 hours of the last shift of each calendar week. Payment is due within 30 days of receipt of the invoice.
