You may use the contact form or email me at karefalc@gmail.com to book me for relief at your hospital. Once dates are confirmed, an independent contractor agreement, practice questionnaire and W-9 form will be emailed or mailed to you. 

An availability calendar is included below. Dates in green are currently available. Dates in red are not available. Availability is subject to change.
